    Parents and guardians, if you have a student in your household who has an immediate family member connected with the U. S. military, please take a few minutes to complete a short form (ENGLISH | SPANISH.) This includes family members serving on active duty, in the National Guard and Reserves, Retired Military, Disabled Veterans or Civil Service Employees. The information on the short survey form is being collected on behalf of the North Carolina General Assembly to ensure that the unique needs of military-connected students are met.
