    Student Medication

    Students requiring the use or possible use of medication (prescription or non-prescription) during the school day must have the Authorization of Medication for a Student at School form filled out and on file before any medication can be administered. The form can be found using the school webpage under the Departments Tab, click Parents: Student Health & Medication Forms.

    All prescription medication must be sent to the Main Office in a container labeled by the pharmacist and must include the following information on the label:

     

    • Student’s full name
    • Name of medication
    • Correct dosage
    • Prescribing doctor’s name & phone number

     

    Medication and paraphernalia shall be kept and administered under the supervision of the Main Office staff. The unauthorized possession and use by students of non-prescription drugs or medication during the school day may result in disciplinary action against the students.
