• User name - First Name, Last Initial, and last 4 of social
• Password - Full Social Security Number with no space or dashes
• Follow steps to enroll or decline and upload appropriate dependent verification where needed.

If you are adding dependent(s) to the NC State Health Plan, dependent eligibility verification is required. Please review the attached list of documents to choose from when verifying your dependent(s) eligibility. Upload the appropriate documentation in the "documents" section of the enrollment.
