EOC Retest Consent:
Starting this school year, students who score Not Proficient on an End-of-Course (EOC) test are eligible for a retest opportunity each year. Families with students who are in Math 1, Math 3, English 2 or Biology this semester should complete this form. This form gives consent for your student to participate in retesting should they need it. This consent will remain in effect until your child graduates or you revoke it in writing. You will be contacted after EOC testing if your child is not proficient and eligible to take advantage of the rest opportunity.
