Important Documents and Information

SHARED HOUSING AFFIDAVITS

1. Print the affidavit forms (linked below), fully complete, and have notarized
2. Scan or Photograph the notarized affidavit along with:
     a. *Two current proof of residence in the resident provider/property owner's name
     b. One current piece of mail in the parent's name going to the address

*Refer to the "Checklist" sheet for approved proof of residence

Both the "Property Provider" AND the "Parent or Legal Guardian" forms must be notarized and submitted together.

(All Documents must show the current date, full name, and full address)

ALL DOCUMENTS SHOULD BE EMAILED TO AOR@abss.k12.nc.us or DELIVERED IN PERSON TO 1712 VAUGHN RD. BURLINGTON, NC.

SHARED HOUSING AFFIDAVITS AND DOCUMENTS SHOULD NOT BE SUBMITTED TO THE SCHOOL.

CONTINUATIONS (REQUEST MUST BE MADE WITHIN 15 DAYS OF THE CHANGE IN DOMICILE)

If there is a change of domicile, the parent/legal custodian may elect the student to remain/continue in the school of enrollment for the remainder of the school year, as long as the parent/legal custodian provides transportation. Notification of the student's domicile change must be reported within fifteen calendar days to the Office of Student Assignment. Please contact the School Administration Office to request a continuation.

TRANSFERS

Out of County transfers will be charged a tuition fee of $2048.00 per school year/per student.  Tuition must be paid in full by August 1 of each year.

Approved Transfers may also result in a 365 athletic ineligibility period for middle/high school students.  Please refer to BOE Policy 4150 for more information.
