End-of-Year (EOY) Retest Consent
If your child does not pass their End-of-Year (EOY) Test, they are eligible for a second attempt. However, a retest cannot be administered without a parent/guardian’s formal consent.

Submit Early: You may complete the form before the initial test date. If your child passes their first attempt, the form will remain on file, and no further action is needed.
